Windber Home Health provides patients with continuity of care-from hospital or other facility to home. Direct patient care, support, and education is provided by Registered Nurses, Certified Nurses Aides, Physical, Occupational and Speech Therapists. Professional services help a patient function as independently as possible in their own home.

Whether an individual is recovering from a recent illness or surgery or is living with a chronic disease, familiar surroundings in the company of family and friends promote a positive healing experience. We understand that homebound patients have special needs, as do their families and caregivers. Our experienced Home Health providers bring a high level of compassion, commitment, and respect as they care for individuals and their families.

Intermittent, part-time skilled nursing and support services are available to qualifying homebound patients. Additionally, a person admitted must be under the care of a licensed physician, have a skilled nursing need, or require rehabilitation services. The patient’s physician must certify the need for Home Care services. Patients are considered homebound if they:

  • Are essentially confined to home
  • Employ considerable and strenuous effort to leave home
  • Are absent from home infrequently for short duration or to receive medical attention

An individualized plan of care is formulated for each patient. Our home health team partners with family members and other caregivers to provide the best, most comfortable healing environment for all. Most Home Care services are covered by insurance. Co-pays or deductibles may apply. If you do not have insurance other financial arrangements are available.
