Community Health Needs Assessment Survey

As part of Chan Soon-Shiong Medical Center at Windber’s (CSSMCW) commitment to creating a healthier community, we are undertaking a Community Health Needs Assessment (CHNA). The purpose of the CHNA process is to identify and prioritize the most important community needs and priorities and to develop and implement action plans to improve the health of the community. We have retained Strategy Solutions, Inc. to assist us with the CHNA process.

We would like to collect input from a broad and diverse representation of our community to help inform our assessment. Therefore, we invite all community members to please take a few minutes to complete a brief online survey to provide input on important health topics and issues in our community.

The survey should not take you more than 10 minutes to complete and your input is anonymous. The survey can be accessed at:

On behalf of the team at CSSMCW thank you for providing us with this valuable information. The results enable us, as well as other community providers, to more strategically identify community health priorities, develop interventions, and commit resources to improve the health status of the region.
